Study Questions Discussion

The federal government recently implemented Secure Communities, a program of the Department of Homeland Security where local police departments are connected with DHS to identify illegal immigrants who have committed crimes and deport them. Unfortunately, in working to identify and deport illegal immigrants, several American citizens have been improperly detained before being identified as citizens and released. How can the rights of Americans with certain ethnic backgrounds be protected as the government enforces U.S. immigration laws?
What did Brown v. Brown of Education accomplish? Has it failed to fulfill its promise? Can or should U.S. Courts be the agents to bring about social change in society?
Most Americans seem to support the principle of equal opportunity for all groups, but there is significant disagreement over government’s role in ensuring equal outcomes. What is the difference between equality of opportunity and equality of outcome in the context of women’s rights? Racial equality?
One of Title IX’s purposes was to create equality in intercollegiate athletics. Has it achieved equality between the sexes? Should government play a role in ensuring specific opportunities for extracurricular activities for students?

Somatic Symptom Disorders

The illness and the dysfunctional focus and behavior around the illness can assume a central role in the person’s life.
Somatic symptom disorders were originally thought of as “hysterical,” without legitimate medical causation, or as hypochondriasis. Though thinking has changed, negative judgments about unfounded illnesses can still be attached to individuals with these disorders. The boundary between medical and emotional problems can be further blurred. In some cases, an individual labeled with one of these illnesses may simply be experiencing a developing medical condition that has not yet been well defined. For all of these reasons, social workers need to take particular care in diagnosing somatic symptom disorders and in providing a fully biopsychosocial and multidisciplinary approach.
In this Assignment, you describe what that approach might look like for one client.
To prepare:
Imagine that Jennifer Brea, whose TEDTalk (TED Conferences, LLC, 2016) you watched, is referred to you for ongoing supportive therapy when her psychiatry consultant decides that she does not have a conversion disorder. Despite the psychiatrist’s opinion, her primary care physician ignores that consult and labels Jennifer with the conversion disorder anyway. Be sure to investigate what the ‘conversion’ diagnosis means when responding. https://drive.google.com/file/d/1MP2_L1uO01McUYFhM…
Record your presentation using Personal Capture (record audio, video, and screen). See Kaltura Media Uploader in left-hand navigation menu in course for directions on recording and uploading media.
By Day 7
Submit a 5-minute recorded PowerPoint (5–7 slides) in which you address the following:
Explain in a concise professional manner how you would conduct your first meeting with Jennifer. Identify specific steps you would take to understand her circumstance and needs.
Explain how you would proceed with her medical team in terms of advocacy for her as a client believed to have this condition.
Explain why you would need to take a biopsychosocial approach to her ongoing care.
Explain what social, family, vocational, Internet, and medical supports you would explore to help with her longer-term stabilization.
Analyze the controversy in diagnosing a mental disorder based on unexplained physical symptoms. Within your analysis, consider how power and privilege influence who provides the diagnoses and which groups are more likely to be diagnosed with certain disorders. Explain your thoughts on this debate.
Support your presentation with research and references to scholarly literature.
Include a transcript and/or edit closed captioning in your presentation to ensure your presentation is accessible to colleagues of differing abilities.

The Complexity of Eating Disorder Recovery in the Digital Age

These behaviors have drastic impacts on health. In clinical practice, social workers need to know about the resources available to clients living with an eating disorder and be comfortable developing interdisciplinary, individualized treatment plans for recovery that incorporate medical and other specialists.
For this Discussion, you focus on guiding clients through treatment and recovery.
To prepare:
Review the Learning Resources on experiences of living with an eating disorder, as well as social and cultural influences on the disorder.
Read the case provided by your instructor for this week’s Discussion
Psychiatry Online | DSM Library (waldenulibrary.org)
What happens after treatment? A systematic review of relapse, remission, and recovery in anorexia nervosa | Journal of Eating Disorders | Full Text (biomedcentral.com)
Behavioural eating disorders – ScienceDirect (waldenulibrary.org)
From diagnosis to social diagnosis – ScienceDirect (waldenulibrary.org)
By Day 3
Post a 300- to 500-word response in which you address the following:
Provide the full DSM-5 diagnosis for the client. Remember, a full diagnosis should include the name of the disorder, ICD-10-CM code, specifiers, severity, and the Z codes (other conditions that may be a focus of clinical attention). Keep in mind a diagnosis covers the most recent 12 months.
Explain the diagnosis by matching the symptoms identified in the case to the specific criteria for the diagnosis.
Explain why it is important to use an interprofessional approach in treatment. Identity specific professionals you would recommend for the team, and describe how you might best utilize or focus their services.
Explain how you would use the client’s family to support recovery. Include specific behavioral examples.
Select and explain an evidence-based, focused treatment approach that you might use in your part of the overall treatment plan.
Explain how culture and diversity influence these disorders. Consider how gender, age, socioeconomic status, sexual orientation, and/or ethnicity/race affect the experience of living with an eating disorder.
Note: You do not need to include an APA reference to the DSM-5 in your response. However, your response should clearly be informed by the DSM-5, demonstrating an understanding of the risks and benefits of treatment to the client. You do need to include an APA reference for the treatment approach and any other resources you use to support your response.

Transference and Countertransference

Understanding transference and countertransference is crucial to a healthy therapeutic relationship. Both transference and countertransference can be evident in any client–therapist relationship, but are especially important in working with children because of a common instinct among adults to protect and nurture the young. The projection or relocation of one’s feelings about one person onto another, otherwise known as transference, is a common response by children (Gil, 1991). Countertransference, a practitioner’s own emotional response to a child, is also common.
For this Discussion, review the Malawista (2004) article.
By Day 4
Post your explanation why transference and countertransference are so common when working with children. Then, identify some strategies you might use to address both transference and countertransference in your work with children.
Support your posts with specific references to the Learning Resources. Be sure to provide full APA citations for your references.By Day 6
Respond to at least two colleagues who identified strategies different from your own by proposing alternative strategies.
Colleague 1: Kristine
Transference and counter-transference
Transference occurs when those we serve transfer their positive regard onto the person they put in the role of importance or good in their life. This person could be the therapist, doctor, school counselor, or teacher. Counter transference happens when the therapist, doctor, school teacher, or counselor adopts the rescue fantasy, taking their clients emotional status unto themselves, and becoming their knight in shining armor. According to Malawista (2004), transference and counter-transference happen when social workers assume the role of “right and good” in a child’s current situation of “right and bad”, being the parents. The child can put the social worker in that place and depending on the child’s story, it would be difficult to not be upset or angry on behalf of the child and the situation they suffered through.
Strategies to keep this from happening
This first thing that comes to mind is to remember that the story the child tells you is only half of the equation. While we may want to believe everything they say, sometimes the story comes out backwards and sideways. Other ways to keep the story concise and to the point is through motivational interviewing, play therapy, and drawing activities. For some children pictures tell the story better than words on a page, or questions asked.

Play Therapy

Children often reenact trauma through repetitious play in order to establish mastery over their emotions and integrate experiences into their history on their own terms. Through the use of toys and props, children may naturally share their emotions and past experiences without feeling the pressure they might encounter with traditional talk therapy.
For this Discussion, review the course-specific case study for Claudia and the Chiesa (2012) and Taylor (2009) articles.
By Day 3
Post an explanation of ways play therapy might be beneficial for Claudia. Using the insights gained from the articles, describe ways you might have worked with Claudia to address her fears and anxiety related to the mugging she witnessed.
Support your posts with specific references to the Learning Resources. Be sure to provide full APA citations for your references.By Day 5
Respond to at least two colleagues by expanding on evidence in support of play therapy.
Colleague 1: Melinda
Claudia’s presenting problem is her fear and “needy” behavior after witnessing a mugging in her community (Plummer, 2014). It was report Claudia has high anxiety when leaving the house and being at school (Plummer, 2014). Within Claudia’s case it would be beneficial to use play therapy for a number of reasons. Talmadge explains there are five beneficial reasons to use play therapy: “1. Children communicate through their play; 2. Children are not miniature adults; 3. Children will participant if its fun; 4. It may appear like its just play but there is so much more going on; 5. Play therapy is a developmentally appropriate practice” (2017). With Claudia’s age, emotions would best be expressed through play therapy. “A primarily nonverbal method of intervention, the “work” is done through the sand material and the carefully selected toys the client uses to construct and sometimes to play out his or her world. Because sandboxes are familiar to most children, sand play is not likely to be threatening and more likely to be a safe way to express what may seem to be unacceptable feelings and impulses” (Taylor, 2009). When using play therapy with Claudia, the social worker can use the toys to his display how she feels in another way. Claudia also can express her feelings and anxiety through using sand or other play. With young kids like Claudia, using dolls, babies, Barbie’s, or action figures can also help them reenact the trauma. Within the stages of play therapy she can first address the problem, and goals with the miracle question and then work on the feedback of how to move on (Taylor, 2009).
