Somatic symptom disorders were originally thought of as “hysterical,” without legitimate medical causation, or as hypochondriasis. Though thinking has changed, negative judgments about unfounded illnesses can still be attached to individuals with these disorders. The boundary between medical and emotional problems can be further blurred. Understanding transference and countertransference is crucial to a healthy therapeutic relationship. Both transference and countertransference can be evident in any client–therapist relationship, but are especially important in working with children because of a common instinct among adults to protect and nurture the young. The projection or relocation of one’s feelings about one person onto another, otherwise known as transference, is a common response by children (Gil, 1991). Countertransference, a practitioner’s own emotional response to a child, is also common.

Transference and counter-transference
Transference occurs when those we serve transfer their positive regard onto the person they put in the role of importance or good in their life. This person could be the therapist, doctor, school counselor, or teacher. Counter transference happens when the therapist, doctor, school teacher, or counselor adopts the rescue fantasy, taking their clients emotional status unto themselves, and becoming their knight in shining armor. According to Malawista (2004), transference and counter-transference happen when social workers assume the role of “right and good” in a child’s current situation of “right and bad”, being the parents. The child can put the social worker in that place and depending on the child’s story, it would be difficult to not be upset or angry on behalf of the child and the situation they suffered through.
Strategies to keep this from happening
This first thing that comes to mind is to remember that the story the child tells you is only half of the equation. While we may want to believe everything they say, sometimes the story comes out backwards and sideways. Other ways to keep the story concise and to the point is through motivational interviewing, play therapy, and drawing activities. For some children pictures tell the story better than words on a page, or questions asked.

Children often reenact trauma through repetitious play in order to establish mastery over their emotions and integrate experiences into their history on their own terms. Through the use of toys and props, children may naturally share their emotions and past experiences without feeling the pressure they might encounter with traditional talk therapy.

Claudia’s presenting problem is her fear and “needy” behavior after witnessing a mugging in her community (Plummer, 2014). It was report Claudia has high anxiety when leaving the house and being at school (Plummer, 2014). Within Claudia’s case it would be beneficial to use play therapy for a number of reasons. Talmadge explains there are five beneficial reasons to use play therapy: “1. Children communicate through their play; 2. Children are not miniature adults; 3. Children will participant if its fun; 4. It may appear like its just play but there is so much more going on; 5. Play therapy is a developmentally appropriate practice” (2017). With Claudia’s age, emotions would best be expressed through play therapy. “A primarily nonverbal method of intervention, the “work” is done through the sand material and the carefully selected toys the client uses to construct and sometimes to play out his or her world. Because sandboxes are familiar to most children, sand play is not likely to be threatening and more likely to be a safe way to express what may seem to be unacceptable feelings and impulses” (Taylor, 2009). When using play therapy with Claudia, the social worker can use the toys to his display how she feels in another way. Claudia also can express her feelings and anxiety through using sand or other play. With young kids like Claudia, using dolls, babies, Barbie’s, or action figures can also help them reenact the trauma. Within the stages of play therapy she can first address the problem, and goals with the miracle question and then work on the feedback of how to move on (Taylor, 2009).
